Complete online ethics training available from the Collaborative Institutional Training Initiative (CITI) or NIH’s Protecting Human Research Participant’s training.

Review sample consent forms and guidelines: http://cuhs.harvard.edu/, in preparation for completing the necessary application materials.

Complete the Committee on the Use of Human Subjects’ Application Form.

Note that Human Subject Research may require additional approval beyond that given by the IRB. If your research will include biohazardous materials, radioactive materials, human embryonic stem cells, vertebrate animals, etc. please reference the Office of the Vice Provost’s for Research Policy & Compliance website for additional information.
